### **Responsibilities:**
1. **Patient Assessment:**
– Conduct comprehensive patient assessments to determine nursing care needs.
– Monitor vital signs and assess the patient’s physical and emotional condition regularly.
– Develop individualized care plans based on patient needs and rehabilitation goals.
2. **Patient Care:**
– Administer medications, wound care, and other prescribed treatments.
– Assist with physical therapy and rehabilitation exercises as directed by therapists or physicians.
– Provide support with daily living activities such as bathing, dressing, and mobility.
– Monitor progress and adjust care plans in collaboration with the healthcare team.
3. **Collaboration with Healthcare Team:**
– Work closely with physical therapists, occupational therapists, speech therapists, and physicians.
– Report changes in patient status and progress to other healthcare providers and family members.
– Participate in multidisciplinary meetings to discuss patient progress and adjust treatment plans accordingly.
4. **Patient and Family Education:**
